Aging in Place

The function of this committee is to take a leadership role in developing programs that will facilitate the ability of seniors in our community to remain as independent as their medical status and interests allow. This committee often works in partnership with CentraState’s SeniorsFirst program.
A collaboration between CentraState, the Visiting Nurses Association of Central Jersey and the Monmouth County Office on Aging provides “Telecare Connections,” an internet-based program that offers daily, web-based health education along with a communications network.

This program, funded by grants from Robert Wood Johnson Foundation and Verizon, allows participants using a computer to access the service which provides a medication inventory with reminders, remote monitoring of vital signs, nutrition plans and more. Clients can view health education programs, participate in fitness activities, view live web casts of local events, and send and receive e-mail, among other functions. Nurses and care managers can conduct individual “tele-visits,” monitor vital signs, and provide education, support and training.
